In order for us to provide the best care for you, it is very important that you bring the following items to your visit:
- A list of your current medications
- Recent x-rays films and lab work
- Insurance information and card
- Medical records from prior visits that you think may be helpful
- Pathology report if a biopsy was performed

Registration / Patient Forms
Before your first appointment you will be receiving a Metro Urology New Patient Packet. This packet includes information about the day, time, and location of your appointment, instructions on what to bring to your appointment, directions to our clinic location, as well as important paperwork. The registration and the patient history forms may also be downloaded. Please bring the completed paperwork to your initial visit with us.
